The Sample Release of Confidential Information form serves as an example of a formal agreement between a client and a coach who wants to use a recording of a session in their ICF credential application. Its primary purpose is to obtain the client’s consent to record and release a coaching session’s audio and written transcript. This transcript is required when applying for a credential – performance assessors will listen to and evaluate the session to ensure a coach’s competency through a real coaching session.
