This is an official ICF Mentor Coaching Session Observation Form designed to support individual mentor coaching at the Master Certified Coach (MCC) credential level. The form supports reflective learning, professional growth, and verification for the ICF credentialing process.
Mentor coaches with the ICF Mentor Coach Specialization (MCS) may use this structured documentation tool to:
- Observe and evaluate a coach’s skills during a live or recorded coaching session.
- Document evidence-based feedback.
- Assess performance against the ICF Core Competencies and ICF Code of Ethics at the MCC level.
To support meaningful feedback and consistent evaluation, mentor coaches document specific evidence — such as timestamps, quotes, and observed behaviors — for each competency rating.
In addition to competency ratings, the form includes sections for identifying strengths, highlighting opportunities for development, and providing written feedback that encourages reflection, learning, and continued growth.
The primary purpose of this form is to support the coach’s development. Mentor coaches should review the completed form with their mentor coaching client to encourage reflection and learning. A completed form may also be requested if an MCC application is selected for audit.
By providing a structured framework for observation and feedback, this form promotes consistency, rigor, and transparency while supporting coaches in refining their practice, demonstrating mastery of the ICF Core Competencies, and progressing toward MCC credentialing.
